DATA SECURITY

You are in control of who you share your information with and when. At no time will information be sent without your authorization and only the information you’ve chosen to share is shared.
When you choose to share your Medical Report, the information is transferred through a secure link sent by email to the person or facility you want to share the information with. The link is only available for 24 hours, at which time it will expire. The link allows the recipient to download the information you have shared through CONTROL.
CONTROL Your Health Record™ powered by EMTeLink® uses secure Microsoft Azure data centers located in the United States. Not only is the system HIPAA compliant, it uses Military Grade FBI encryption to further secure your data.

HOW TO CREATE AN ACCOUNT

From the Control.health website select PRICING from the top menu bar. Choose the plan that works for you, click SUBSCRIBE NOW, and follow the prompts to set up your account.

NOTE: You will need an email address, a password, and your credit card for payment.
You will receive a confirmation email with a link to activate your account. You must click on the activation link in the confirmation email to activate your account.

NOTE: The email will come from support@emtelink.com. Please check your spam folder if you have not received the email.

LOGGING IN AND PASSWORDS

From your computer: Go to Control.health website select and click LOG IN located at the top right of the screen. This will take you to a login screen where you will enter your EMAIL ADDRESS and PASSWORD.

NOTE: We recommend bookmarking this page for future access.

Using the CONTROL App: The login screen will display when you open the CONTROL app. Enter your EMAIL ADDRESS and PASSWORD and click LOG IN.

NOTE: Get the CONTROL App on the Apple App Store or Google Play Store by searching "CONTROL Your Health Record".
From your computer: There are two ways to log out of the system from the website. The first way is to go to the menu bar to the left of the screen and click LOGOUT. The second way is to click on your name at the top right of the screen and select LOGOUT from the drop-down menu that appears. Using the CONTROL App: Click LOGOUT from the menu bar at the bottom of the screen.
Your user ID is the email address that you used during sign-up. If you previously had a User ID, CONTROL has been simplified so that you no longer have to create and remember a User ID.

FAMILY ACCOUNTS

Select FAMILY from the menu bar on the left of the screen. Select ADD NEW USER and follow the prompts. Be sure to let the family member know the EMAIL ADDRESS and PASSWORD that you create for them so they can log in and fill out their information. In the case of a minor child or family member you are caring for, follow the same process as you did when setting up your own Medical Record.
In the case of a family member you are caring for or a minor child, you will have to fill out that person’s profile. First, log into the system as yourself. At the top of the screen, find CHANGE USER and click on the drop-down menu which will display the names of the people associated with the account. Select the user you want. You will then follow the same steps that you followed to fill out your own medical report. By using the CHANGE USER drop-down menu, you can toggle between family members associated with your account.
Yes. The family member should first log in to the system using the login and password that you created for them when setting up the account. Then, click on FAMILY in the menu bar on the left of the screen. This will show a list of family members on the account. Click the word ACTION beside their name, select RESET PASSWORD and follow the prompts.

MOBILE APP

The App is a FREE download from the Apple App Store and the Google Play Store. Use of the App is included with your CONTROL membership.
Click on the App icon on your smartphone or tablet. Use the same EMAIL ADDRESS and PASSWORD that you used to log in to the desktop site. Click LOG IN.
The App allows you to add information to your medical report, securely share records, request records from doctor’s offices and medical facilities, view your records, manage family members on your account, and notify your emergency contacts.

PAYMENT AND BILLING

No. Your credit card will be charged monthly.
You will be charged monthly based on the day that you originally signed up for the service. If you sign up on the 10th of the month, your credit card will be charged on the 10th of every month.
We use PayPal Payments Pro to accept and process credit card payments. Therefore,we are able to accept Visa, MasterCard, American Express, and Discover.
You do not have to have a PayPal account. We use PayPal Payments Pro to accept and process credit card payments but you do not have to have a PayPal account to use this service. However, you will be given the option to sign up for a PayPal account when your first payment to CONTROL is processed.
